I understand that there is risk involved in speed skating. I hereby authorize emergency medical or surgical treatment for myself and/or my daughter/son/ward if such treatment is required while I or my daughter/son/ward is participating in Speed Skate PEI or Speed Skating Canada activities. In consideration of your accepting this registration, I hereby, for myself, my heirs, executors, administrators and assigns, waive and release any rights and claims for damages I may have against Speed Skating Canada, Speed Skate PEI Inc., the Eastlink Centre, Capital Area Recreation Inc. and/or the City of Charlottetown, its agents, officers, or members, for all and any injuries suffered by me during said program to be held during the month of April, 2019 at the Eastlink Centre and BellAliant Centre.

Skate Rental Agreement *

I agree to maintain the provided rental speed skates (boots and blades) in good condition with thorough drying after each use, proper use of hard and soft skate guards and proper storage. I agree to return the rental skates at the end of the program on a timely basis. If the skates are lost or damaged through negligence or theft, I agree to pay Speed Skate PEI Inc. for replacement skates of comparable quality.
